Booking an Appointment

Please note:

  • If you are unable to find a booking prior to your interview or event, please call 0437 771 947.
  • Young people aged under 18 years of age, must be accompanied to their appointment with either a parent, guardian or trusted adult (could be a school teacher or referral partner they are working with). 
  • If you would like to make a group booking, please call 0437 771 947.

Booking on behalf of a Client

Referral Providers, if you would like to book on behalf of your Client, please follow these steps:

  1. Using the booking form below, select the type of service your client requires.
  2. Select an appropriate date and time.  If a suitable time is not available, contact us on 0437 771 947.
  3. Fill in your client’s contact details (name, address, email and phone number).
  4. Fill in your details (Referral Organisation, your name and contact details).
  5. Advise if you are attending with your client.
  6. Submit the booking.
  7. Optional – if you wish to assist your client to fill out the referral form, please download a copy WORD Version  OR  PDF Version
